This is a template. Ask a lawyer to adapt it to the laboratories, hosts, and retention periods you actually use before you collect real enquiries.
Who we are
Kinvera uses this website to take appointment requests. The public contact details are hello@kinvera.example and +234 800 000 0001.
What the forms collect
Booking and contact forms ask for your name, phone number, email address, the kind of test you are asking about, and a short message. Do not send medical records or a child’s history through the form. A coordinator will ask only for what the laboratory needs, and only from people who have agreed.
Why we collect it
We use the details to reply, book a room, and keep a record of the request. The record is stored in this website’s database. If you enter an alert email in the dashboard, a copy of the enquiry is also emailed there.
How long we keep it
Delete enquiries you no longer need from the dashboard. Set a retention period with your lawyer and write it here. Sample text does not choose that period for you.
Sharing
We do not sell enquiry lists. A laboratory receives a person’s details only after that person has agreed to a test and only as that test requires. Your hosting company processes the database because the site runs on their server.
Your requests
You can ask what we hold about you, ask for a correction, or ask us to delete an enquiry. Write to hello@kinvera.example. If a laboratory already has a sample, contact that laboratory as well.
